What you'll need

Proof of Identification: Valid photo-bearing government-issued ID (front and back) such as Philippine Identification System (Philsys ID), Passport, Driver's License, SSS ID, etc. Supplementary applicants who are studying may submit a valid school ID
For Foreigners: Copy of Employment Contract or Certificate of Employment, Letter from Embassy (if Embassy official), Valid VISA and work permit; Alien Certificate of Registration (ACR) or Immigrant Certificate of Registration (ICR) or ACR-I; or valid passport
Proof of Income - Employed Applicants: Photocopy of latest Income Tax Return (ITR) duly stamped as received by BIR or BIR Form 2316 signed by employer's authorized representative (MANDATORY); Plus latest full-month payslip(s) or Original Certificate of Employment or copy of last two months credit card statements if existing card at least 1 year on books
