What you'll need

Valid photo-bearing identification document (front and back) - Government-issued ID such as Philippine Identification System/Philsys ID, Passport, Driver's License, or SSS ID for Filipinos
For foreign applicants: Copy of Employment Contract or Certificate of Employment, Letter from Embassy if applicable, and valid documents (VISA with work permit, ACR/ICR, or valid passport with appropriate VISA)
Photocopy of latest Income Tax Return (ITR) duly stamped by Bureau of Internal Revenue or BIR Form 2316 signed by employer's authorized representative (MANDATORY for employed applicants)
For employed applicants: Latest full-month payslip(s), Original Certificate of Employment, or last two (2) months credit card statements if existing cardholder for at least 1 year
For self-employed applicants: Photocopy of latest ITR stamped by BIR and Audited Financial Statements for at least two (2) years (MANDATORY)
For self-employed applicants: Photocopy of Business Name registration (MANDATORY) - DTI Registration for Single Proprietorship or SEC Registration for Partnership/Corporation
Last 3 months' bank statements (OPTIONAL for self-employed applicants)
