Applicant must be a Filipino citizen or a foreigner who is a resident of the Philippines
Supplementary cardholders must be at least 13 years old

What you'll need

Proof of identification: Government-issued ID (Philsys ID, Passport, Driver's License, SSS ID, etc.) for principal and supplementary applicant
For studying supplementary applicants: valid school ID accepted
For foreigner applicants: Employment Contract or Certificate of Employment, Letter from Embassy (if applicable), and valid VISA with work permit or Alien/Immigrant Certificate of Registration (ACR/ICR) or valid passport with appropriate VISA
Proof of income for employed applicants: Latest ITR duly stamped by BIR or BIR Form 2316 signed by employer (mandatory) plus latest payslip or Certificate of Employment or credit card statements
Proof of income for self-employed applicants: Latest ITR duly stamped by BIR and Audited Financial Statements for at least 2 years (mandatory), Business Name registration, DTI or SEC registration, and optional last 3 months bank statements
