What you'll need

Valid photo-bearing identification document (front and back) for both primary and supplementary applicants—Government-issued ID such as Philsys ID, Passport, Driver's License, SSS ID, etc., or valid school ID for supplementary applicants who are studying
Photocopy of latest Income Tax Return (ITR) duly stamped as received by Bureau of Internal Revenue (BIR) or BIR Form 2316 signed by employer's authorized representative (mandatory for employed applicants)
For employed applicants: any of the following—latest full-month payslip(s), Original Certificate of Employment indicating status, service tenure, and compensation breakdown, or copy of last two (2) months statements of account of credit card at least 1 year on books
For self-employed applicants: Photocopy of latest ITR duly stamped by BIR and Audited Financial Statements for at least two (2) years (mandatory)
For self-employed applicants: Photocopy of registration of Business Name (mandatory); DTI Registration for Single Proprietorship or SEC Registration for Partnership/Corporation
For self-employed applicants: Last 3 months bank statements (optional)
For foreigners: Copy of Employment Contract or Certificate of Employment
For foreigners: Letter from Embassy (if Embassy official)
For foreigners: Any one of the following—Valid VISA and work permit; Alien Certificate of Registration (ACR) or Immigrant Certificate of Registration (ICR) or ACR-I; or valid passport with Special Investors Resident VISA, Special Non-Immigrant VISA for PEZA investors and employees, or VISA with EO226
