What you'll need

Valid photo-bearing government-issued ID (Philippine Identification System, Passport, Driver's License, SSS ID, etc.) for both principal and supplementary applicants
Latest Income Tax Return (ITR) duly stamped by Bureau of Internal Revenue (BIR) or BIR Form 2316 signed by employer's authorized representative (MANDATORY)
For employed: latest full-month payslip(s) OR original Certificate of Employment with status, tenure, and compensation breakdown OR last two months' credit card statements (if existing cardholder for at least 1 year)
For self-employed: Audited Financial Statements for at least two years (MANDATORY), business registration (DTI for sole proprietorship or SEC for partnership/corporation)
