For foreigners: Copy of Employment Contract or Certificate of Employment, Letter from Embassy if applicable, and any one of the following - Valid VISA and work permit, ACR/ICR, or valid passport with Special Investors Resident VISA, PEZA VISA, or EO226 VISA
Latest Income Tax Return (ITR) duly stamped by BIR or BIR Form 2316 (mandatory for employed applicants)
For employed applicants: latest payslip, Certificate of Employment, or last two months credit card statements
For self-employed applicants: Latest ITR and Audited Financial Statements for at least two years (mandatory)
For self-employed applicants: Business Name registration (mandatory), DTI Registration or SEC Registration
For self-employed applicants: Last 3 months bank statements (optional)
