Filipino citizen or foreign resident of the Philippines
Supplementary cardholders must be at least 13 years old

What you'll need

Valid photo-bearing government-issued ID (front and back) for Filipino applicants
For foreigners: Employment Contract or Certificate of Employment, or Letter from Embassy, plus valid VISA and work permit or ACR/ICR or passport with special VISA
For employed applicants: Latest Income Tax Return (ITR) with BIR stamp or BIR Form 2316 (mandatory) plus payslip or Certificate of Employment or last two credit card statements
For self-employed applicants: Latest ITR with BIR stamp and Audited Financial Statements for at least 2 years (mandatory), Business Name registration, DTI or SEC Registration, and optionally last 3 months bank statements
