Age 21 to 60
Principal cardholder age 21-60 years; supplementary cardholder at least 13 years old
Must be a Filipino citizen or foreigner who is a resident of the Philippines

What you'll need

Valid photo-bearing government-issued ID (Philippine Identification System, Passport, Driver's License, SSS ID, etc.) for principal applicant
For supplementary applicants: valid school ID if studying, or same ID requirement as principal
For foreign applicants: Copy of Employment Contract or Certificate of Employment, Letter from Embassy if applicable, and valid VISA/work permit or ACR/ICR or passport with special VISA
Photocopy of latest Income Tax Return (ITR) duly stamped by Bureau of Internal Revenue (BIR) or BIR Form 2316 signed by employer (mandatory for employed)
Latest full-month payslip(s), Certificate of Employment indicating status and compensation, or last two months credit card statements (if card at least 1 year on books)
For self-employed: Latest ITR stamped by BIR and Audited Financial Statements for at least two years (mandatory)
For self-employed: Business registration documents (MANDATORY)
For self-employed: Last 3 months bank statements (optional)
