Who can apply

Age 21 to 60
Principal cardholder: 21–60 years old
Supplementary cardholder: at least 13 years old
Must be Filipino citizen or foreigner resident of Philippines

What you'll need

Valid photo-bearing identification (Government-issued ID, Passport, Driver's License, SSS ID, Philsys ID, etc.) for both principal and supplementary applicant; school ID acceptable for studying supplementary applicants
Latest Income Tax Return (ITR) duly stamped by BIR or BIR Form 2316 signed by employer (MANDATORY for employed)
For employed: latest full-month payslip, Certificate of Employment, or last two months' credit card statements (1+ year old)
For self-employed: latest ITR stamped by BIR and Audited Financial Statements for 2+ years (MANDATORY), Business Name registration, DTI or SEC registration, and optional last 3 months' bank statements
For foreigners: Employment Contract or Certificate of Employment; Letter from Embassy if embassy official; Valid VISA and work permit, ACR/ICR, or passport with Special Investor VISA, PEZA VISA, or EO226 VISA
