Applicant must be a Filipino citizen or a foreigner who is a resident of the Philippines

Valid photo-bearing government-issued identification (front and back) for principal applicant
Valid school ID for supplementary applicants who are studying
For foreigners: Employment Contract or Certificate of Employment, valid VISA and work permit, ACR/ICR, or valid passport with special VISA
Latest Income Tax Return (ITR) duly stamped by BIR or BIR Form 2316 (mandatory for employed applicants)
Latest payslip or Certificate of Employment indicating status, service tenure, and compensation breakdown
For self-employed: Latest ITR and Audited Financial Statements for at least 2 years (mandatory), DTI/SEC Registration, last 3 months bank statements
