What you'll need

One photo-bearing government-issued ID with identification number or date of birth
Proof of Income (Pay Slip, BIR Form 2316, Original Certificate of Employment, or Latest ITR 1700/1701 with BIR or Bank Stamp)
For Foreign Nationals: Valid passport or ACR, and proof of local residency (ICR, ACR-I-Card with Permanent Visa status, Visa and Work Permit valid 1+ year, SRIV, or SRRV)
