Must have Tax Payer's Identification Number (TIN) and SSS/GSIS/UMID Number

What you'll need

One photo-bearing government-issued ID with identification number or date of birth
Proof of Income (Pay Slip, BIR Form 2316, Certificate of Employment, or ITR with BIR/Bank Stamp)
For foreign nationals: valid passport or ACR with proof of local residency (ICR, ACR-I-Card, Visa and Work Permit, SRIV, or SRRV)
